Information about comorbidities and vital signs for each CDCR resident from January 1, 2016 to the present.

General comments

This dataset is comprised of monthly observations (i.e., snapshots) describing the resident's comorbidities and risk scores at that point in time. These observations are automated abstractions of diagnostic codes, procedure codes, medications, vital signs, and laboratory results. Only observations made on the last day of each month are retained; observations on other days of the month are not. Some records may span just a single month, i.e., the StartDate and the EndDate are the same date, whereas other records may span multiple months, i.e., the StartDate and EndDate are different. In cases where a record spans multiple months, the comorbidity and severity for that record apply to the last day of each month within that range. Consider the following set of records, which comprise all comorbidity=HTN records for a single resident between January 1, 2016 and the present:

Based on the the first records we can say that Resident 123 was observed to have HTN-MED on:

Because observations are only made on the last day of each month, we cannot say for certain whether Resident 123 was observed to have HTN on any other days of months listed above. However, we can say for certain is that Resident 123 was not observed by CDCR to have HTN on the last day of any other month between January 2016 and the most recently completed calendar month.

Finally, whereas comorbidity values are only provided for end-of-month dates where they were observed, the general clinical risk score (also included in this dataset) is provided for every end-of-month date that a resident was in CDCR custody. Thus, if the last day of a month between January, 2016 and the most recently completed calendar month is not included in the general clinical risk score records of a given resident, that means that resident was not in the custody of CDCR at the end of that month.

Comorbidity

The name of the specific comorbidity or risk score. Comorbidities and risk scores are determined via daily automated abstraction of diagnostic codes, procedure codes, medications, vital signs, and laboratory results from CDCR's electronic health record and from processed outside health care provider claims. Only the values calculated on the last day of each month are reported in this dataset. If a resident is not in CDCR custody on the last day of a given month, no obesrvation for that month is provided. Criteria for each comorbidity are described in Supplemental/Comorbidity criteria. 'Note:' The general clinical risk score is available for every end-of-month day that a resident was in CDCR custody. The Covid weighted risk score is available for every end-of-month day that a resident was in CDCR custody since May 31, 2020. For all other values, only periods of time during which a resident had that comorbidity are included in the dataset.

Severity

For risk scores, the value of the risk score. For comorbidities, the risk classification based on the criteria in Supplemental/Comorbidity criteria.

StartDate

The first snapshot date for which the combined comorbidity and severity level apply. StartDate is always the daily observation made at the end of a given month. The given comorbidity+severity may have started earlier in the month; however, only the daily observation made at the end of the month can be provided.

EndDate

The last snapshot date for which the combined comorbidity and severity level apply. EndDate is always the daily observation made at the end of a given month.
